An unannounced monitoring inspection was conducted on Thursday, February 27, 2020 to determine the program?s compliance with licensing standards. The licensing inspector was on site from approximately 10:00 AM until 3:00 PM. The following ratios were observed with a total of 49 children present during the inspection:
Infants - 9 children with 3 staff
Toddlers - 6 children with 2 staff
Twos - 17 children with 3 staff
Preschool - 9 children with 2 staff
Pre-K - 8 children with 1 staff
School Age were not present during the inspection.
The children and staff were engaged in a variety of activities while the licensing inspector was on the premises. Children were observed playing outside, playing with toys, listening to music, looking at books, learning about dinosaurs, playing a memory game, playing in the sensory table, having their diapers changed, taking bathroom breaks and washing hands, eating lunch and resting at nap time. All areas of the center including classrooms, hallways, kitchen, bathrooms, vehicles and outdoor play area were inspected. The center was equipped with toys and supplies and items were available to the children.
The following information was reviewed: injury reports, injury prevention plan, infant feeding schedules, daily attendance, infant daily sheets, practiced emergency drill logs, liability insurance policy, parent agreements, transportation documents, staff training information, required postings and menu. The center?s first aid kit and emergency supplies were inspected. Medication is administered, and medication and medication authorizations were reviewed. During the inspection, six children's and six staff records were reviewed.

Standard #: 22VAC40-185-130-A Description: Based on review of six children?s records, the center did not ensure that one of six children's records contained current immunization documentation that was dated and signed by a physician, his designee, or an official of a local health department.
Evidence:
The record of Child #3 (start date: 02/25/2020) contained documentation of an immunization record that was not signed by a physician, designee, or an official of a local health department. A member of management confirmed this information during the inspection.Plan of Correction: The parent was notified that an updated copy of the immunizations that contained the required date and signatures needs to be submitted.
Standard #: 22VAC40-185-60-A Description: Based on review of six children's records and interview, the center did not ensure one child's record contained the required information.
Evidence:
The record for Child #3 contained the names, addresses and phone numbers of three people who were listed as emergency contacts. Two of the emergency contacts listed were the parents of Child #3. A member of management called the parent during the inspection and requested an additional emergency contact. Each child's record is required to have at least two designated people to call in an emergency if a parent cannot be reached.Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children.
Evidence:
1. The kitchen door was observed propped open. The cleaning chemicals were located in a cabinet underneath the counter. The cabinet did not contain a locking method.Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the center did not ensure all electrical outlets had protective covers that are of a size that cannot be swallowed by children.
Evidence:
An outlet located above the cots in the Discovery Preschool classroom did not contain protective covers.Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-500-A Description: Based on observation, the center did not ensure children's hands were washed with soap and running water after toileting and staff washed their hands with soap and running water after helping a child use the toilet.
Evidence:
1. Staff #4 was observed changing two diapers in the Preschool classroom. Staff #4 did not wash her hands after changing each child.
2. After changing a child?s diaper, Staff #4 told the child to go wash his hands. The child exited the bathroom and went to the library area located in the Preschool classroom. The child began looking at books and playing with toys. The child was not observed washing his hands.Plan of Correction: We will review the requirements for both staff and children's hand washing procedures with the staff.
