Comments:
An unannounced monitoring inspection was conducted to investigate a self-reported incident regarding the forbidden action of physical discipline and rough handling of a child enrolled in the center. The inspection was initiated with the center director and assistant director on February 4, 2020 beginning at 8:10 AM and ending 9:30 AM. An exit meeting was conducted on February 24, 2020. Staff interviews and a review of documentation and video recording were conducted The allegation regarding the forbidden action was determined to be invalid due to insufficient corroborating evidence to substantiate the allegation.Two violations were cited as a result of the investigation.

Standard #: 22VAC40-185-60-A
Description: Based on a review of one child's record, the center did not ensure that all the children's records had the required information. Evidence: Child A's record was missing the name, address, and phone number for one emergency contact.

Plan of Correction: The child is no longer enrolled at the center. Moving forward, the center will ensure that all children's record are checked to make sure that all the required information is complete. If there is information missing it will be requested from the parents prior to the child's first day.

Standard #: 22VAC40-185-340-A
Description: Based on review of video recording and staff interview, a staff member failed to ensure the care, protection, and guidance of a child at the center. Evidence: Staff A stated that they used their foot to reposition Child A's leg while the child was laying on a cot.

Plan of Correction: The center stated that all staff had completed Conflict Resolution in the Early Childhood Environment and Positive Guidance, Part 1: What Is Positive Guidance and Why Is It Important? through Child Care Education Institute. Staff members also were retrained regarding proper interactions with the children.
