Evidence: Staff #1 confirmed there were no injury prevention procedures available for review.Plan of Correction: The designated center supervisor get the plan and print and post it the center and place copy in the book.
Standard #: 22VAC40-185-70-A Description: Based on review, in one of five staff records reviewed, the licensee did not ensure all the required on site information was available for each staff.
Evidence: There was not documentation on site for Staff #3 that included the name, address and telephone number of a person to be notified in an emergency and information about any health problems which may interfere with fulfilling the job responsibilities, Staff #1 verified this information was not at the center as required.Plan of Correction: The designated center supervisor placed information in the record during inspection.
Standard #: 22VAC40-185-550-M Description: Based on documentation review and staff interviews, the licensee did not ensure the center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and include all required information.
Evidence: Two of two injury reports available for review did not include how the parent was notified of the injury.Plan of Correction: The designated center supervisor will start documenting it today.
