63.2 Facilities & Programs.
An unannounced monitoring inspection was conducted today from 3:45pm through 5:15pm. There were 41 children directly supervised by 4 staff at the beginning of the inspection. The physical plant, programming, menus, 4 staff records, 6 children's records, 11 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. All staff on site are current in CPR/First Aid Training, two staff have current Medication Administration Training, and one staff has Daily Health Observation Training. Children were observed washing hands, eating snack, which consisted of sub sandwiches and juice, and engaging in group gross motor activities in the gym. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on record review, 2 of 6 children's records reviewed did not contain all required information.
Evidence: Child #2's record was missing the father's employer and work telephone number. Child #5's record was missing the father's home address, telephone number, employer and work telephone number.
Plan of Correction: I will be speaking with both parents on Monday morning regarding obtaining what is needed. We will have the information by Wednesday evening.
