Technical Assistance:
Reviewed and discussed linens used for designated rest period and how they are clearly assigned for individual use.

Comments:
An unannounced Monitoring Inspection was conducted on March 2, 2020 from approximately 11:30 am until 4:30 pm. Upon entrance there were 67 children and 15 staff present with children. There were additional staff on site for program and administrative support. The sample size of records reviewed consisted of 7 children's records and 4 staff records. Four medications were reviewed. Children were observed preparing for and having lunch and during story time. The results of the inspection were reviewed and verified with the Center Director during the exit interview. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on documentation review and staff interviews, the licnesee did not ensure to follow it 's own operational responsibilities that shall include, but not be limited to, ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

Evidence: The center did not follow it's own policy related to medication administration.
1)The center's policy states "certified staff members may administer both prescription and non-prescription medications but only with the prior approval of the child's parent or guardian." A center staff, staff #1 with an outdated medication training certificate administered medication on October 22, 2019. The certificate expired July 18, 2018.
2) The center's policy states "if a medication has not been sent home within two days after consent form has expired, WCCC will properly dispose of medication" The medication for child #1 had an expired authorization as of October 7, 2019 and the medication was observed in the center during today's inspection.

Plan of Correction: The Center Director will send home medication today and or dispose of it and communicate with staff about retraining for medication administration and policies.

Standard #: 22VAC40-185-40-J
Description: Based on documentation review and staff interviews, the licensee did not ensure the injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services.

Evidence: The Center Director confirmed there was not an updated annual injury prevention plan.

Plan of Correction: The Center Director will work on the procedure to document injury reports to create annual plan.

Standard #: 22VAC40-185-260-A
Description: Based on documentation review and staff interviews, the licensee did not have documentation of obtaining an annual fire inspection report from the appropriate fire official having jurisdiction.

Evidence: The Center Director confirmed there was not documentation of the annual fire inspection available for review. The most recent fire inspection is dated October 31, 2018.

Plan of Correction: The Center Director will contact the Environmental Health and Safety Staff to obtain the record.

Standard #: 22VAC40-185-330-B
Description: Based on observation, measurements and staff interviews, the licensee did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Falls zones shall not include barriers for resilient surfacing.

Evidence: On the preschool playground, the fall zone for the swings was inadequate as there was a plastic picnic table, a barrier that measured within 10-11 feet from the swings and the swings are required to have a minimum fall zone of 14 feet.

Plan of Correction: The Center Director will move the picnic table.

Standard #: 22VAC40-185-510-A
Description: Based on documentation review and staff interviews, the licnesee did not ensure medication is administered only with written authorization from the parent.

Evidence: The non-prescription medication for child #1 was administered on October 22, 2019 and the medication authorization from the parent had expired on October 7, 2019.

Plan of Correction: The Center Director will ensure the center policy is followed and retrain the staff.

Standard #: 22VAC40-185-510-B
Description: Based on documentation review and staff interviews, the licnesee did not ensure that nonprescription medication shall be administered by a staff member who meets the requirements.

Evidence: The non-prescription medication for child #1 was administered on October 22, 2019 by staff #1 and they do not have current medication administration training. The medication administration certificate for staff #1 expired on July 18, 2018.

Plan of Correction: The Center Director will retrain the staff and verify the staff who have current MAT training.

Standard #: 22VAC40-185-510-N
Description: Based on observation and staff interviews, the licnesee did not ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

Evidence: The medication for child #1 had an expired authorization as of October 7, 2019 and the medication had not been sent home or disposed as required. The medication was observed in the center during inspection.

Plan of Correction: The medication was sent home during inspection.
