Technical Assistance:
Discussed new location of children's belongings.

Each classroom is equipped with individual thermostats. Please remember that inside temperatures must be maintained no lower than 68 degrees.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 55 children with 15 staff were present today. Observation of all 7 classrooms was completed. Morning program today consisted of singing "Head shoulders knees and toes" in class 4 in spanish, children in class 5 were counting numbers in spanish, listing to directions from staff speaking in spanish. The older classes were outside on the playground. Staff were observed strategically placed on the playground to ensure adequate supervision. The center was furnished with new equipment and furnishings throughout the building. Staff were excited with having new programming equipment for the children. Medication/topical ointments were reviewed. A total of 7 children's records and 4 staff records were reviewed. Local County inspections were verified. Previous violations from the November inspection were reviewed and corrections were verified. This inspection began at approximately 10:20am and ended at 2pm. Thank you for your cooperation today. Violations:
Standard #: 22VAC40-185-90--A
Description: Based on child record review, 3 written agreements were incomplete.
Evidence: Child #4, #6 and #7's written agreements did not include authorization for emergency care.

Plan of Correction: The electronic forms changed and an addendum including the authorization did not populate. We will obtain authorizations for all 3 children. We will correct the electronic method of obtaining this information.

Standard #: 22VAC40-185-350-E-3
Description: Based on observation of classroom 5, a 1 to 8 ratio was not maintained.
Evidence: In class #5, the youngest child in the class was 2 years of age. Two staff are assigned to the class. One staff left the classroom to retrieve lunch for the children, leaving one staff with 12 children.

Plan of Correction: We will have an additional staff bring lunch to the classroom daily.

Standard #: 22VAC40-185-510-C
Description: Based on review of medication, the center failed to follow their own medication policy.
Evidence: Child #8 has a parent authorization for an over the counter medication that authorizes the medication for longer than 10 working days.

Plan of Correction: We will return this medication to the parent today.

Standard #: 22VAC40-185-510-L
Description: Based on staff interview, a record of medication given to one child was not maintained.
Evidence: Staff in classroom #5 stated that a cold sore medication was given to Child #8. A record of medication was not completed.

Plan of Correction: We will remind all staff that all medication must be documented when given.
