Comments:
An unannounced Monitoring Inspection was conducted today from 9:43am to 11:12am. There were 39 children from Infants to Pre-K directly supervised by 7 staff. The physical plant, 1 staff records, 10 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing in the multi-purpose room, coloring and reading in small groups. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-60-A
Description: Based on review, the facility did not ensure that children's records were maintained with the required information.
Evidence: Reviewed 10 children's records and found the following:
1) None of the 10 had documentation of emergency contact addresses. Previously cited on 8/29/19.
2) Child 5 and Child 7 did not have documentation of both parents' work numbers.

Plan of Correction: I will go through the files and make sure the contact information is up to date, and those that don't have it we will get it.

Standard #: 22VAC40-185-240-A
Description: Based on review, the facility did not ensure that staff have documentation of orientation training by the end of their first day of assuming job responsibilities.
Evidence: Reviewed 1 staff record and found that that 1 staff member did not have documentation of orientation training.

Plan of Correction: The teacher is returning next week and we will have it signed then.

Standard #: 22VAC40-185-430-B
Description: Based on observation, the facility did not ensure that materials and equipment shall be age and stage appropriate for the children.
Evidence: Observed, accessible, in the 2nd drawer in the Toddler classroom, two rolls of plastic bags that can pose a suffocation risk to the children in the room.

Plan of Correction: Corrected at the time of the visit.

Standard #: 22VAC40-185-520-C
Description: Based on observation, the facility did not ensure that if diaper ointment or cream is used that it is labeled with the child's name.
Evidence: Observed in the Toddler classroom, bathroom, Eucerine not labeled with a child's name, and baby lotion not labeled with a child's name.

Plan of Correction: Corrected at the time of the visit.
