63.2 Child Abuse & Neglect

Technical Assistance:
Discussion on hazardous items, toilets, and attendance.

Comments:
An unannounced monitoring inspection was conducted today from approximately 9:47 am to 1:30 pm. An investigation was conducted today in response to a self-reported incident received by our office regarding the care and protection of an three-year-old. Based upon administration records and interview, the incident is deemed invalid. There were 38 children supervised by 9 direct care staff. The physical plant, 4 staff records, 5 children?s records, children's nutrition, evacuation drills, injury reports, emergency supplies were inspected. Medications were on-site today and records were reviewed. The children were observed in free play, center activities, outdoor play, eating lunch. There were age appropriate materials for all children.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center, inside and outside were not maintained in a safe and operable condition.
Evidence: 1. Outdoors playground area: -There were two tricycles that required maintenance; three were missing a pedal; Picnic fence had splinted wood that could entangle clothing or snag skin.
2. In the classroom for two's, four's and preschool, chipped and peeled paint were located on the window sills and baseboards

Plan of Correction: Staff did remove tricycles when made aware of missing pedal.
I will continue to submit work orders and follow-up.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
In the classroom of three-year-old children, the cleaning supplies for floors were in an closet that was unlocked.

Plan of Correction: Remind staff to lock when the close it.

Standard #: 22VAC40-185-320-B
Description: Based on observation, the center did not ensure toilet paper were within reach of children.
Evidence: In bathroom for three-year-old children, the toilet paper was found on the top shelf that is not within reach for children.

Plan of Correction: Make sure tissue is in the tissue holders.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interview with staff, the diapering surface was not a nonabsorbent surface for diapering or changing .
Evidence: 1. There was a tears on the top corners of the changing pad in the three-year-old classroom making the changing pad a nonabsorbent.

Plan of Correction: Will order a new one.

Standard #: 22VAC40-185-550-M
Description: Based on record review, the center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.
Evidence:
1. 10 out 11 children incident and accident injury reports for January 2020 did not have completed information for time when parents were notified.
2. 5 out of 11 children incident and accident injury reports for January 2020 did not have any future action to prevent recurrence of the injury.
3. 2 out of 21 children incident and accident injury reports for February 2020 did not have time when parents were notified

Plan of Correction: Will review staff on how to complete the form.
