Comments:
An unannounced subsidy health and safety inspection was conducted on 02/10/2020 from 3:25 pm - 5:00 pm. At the time of entrance there were 55 school age children in care with four staff. Children were observed doing homework in the cafeteria. Afterward children were offered the option of outdoor play or inside table games and manipulative toys. Records were reviewed for 11 children.
The vendor's response for the "plan of correction" was not received as of 03/10/2020 and will not appear on this violation notice.
Four staff records were reviewed on 02/13/2020 at the YMCA Greenbrier North Branch where staff records are maintained for the Chesapeake School Based Programs.

Standard #: 22VAC40-665-540-A
Description: Based on record review, the vendor failed to ensure that documentation of the TB screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment.

Evidence:
1. The TB screening for staff 1 was conducted on 06/07/2019. Staff 1 was hired on 05/28/2019 therefore, the TB screening on file was obtained after employment.

Description: Based on staff record review, the vendor failed to ensure that within the first 90 days of employment or subsidy vendor approval all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff.

Evidence:
1. There was no documentation that staff 2 had completed Virginia Preservice Training within 90 days of employment.
a. Staff 2 began work in September 2019 and was observed caring for children during the 02/10/2020 monitoring inspection.
2. There was no documentation that staff 3 had completed Virginia Preservice Training within 90 days of employment.
a. Staff 3 has a hire date of 09/04/2019 and was observed caring for children during the 02/10/2020 monitoring inspection.
3. Administrative staff 5 confirmed that preservice training was not on file for staff 2 and staff 3.
