Comments:
An unannounced monitoring inspection was conducted from 11:00 a.m. - 1:00 p.m. There were 68 children in care being supervised by 14 staff members. Meals are provided from home. The children were observed participating in outdoor play, structured center play, and table top activities. Ten children's records were reviewed and in compliance. Eight staff records were reviewed and in compliance. Three medications were reviewed and in compliance. There are currently two staff members on site who are MAT (Medication Administration Training) certified. The most recent emergency escape drill was conducted 02/28/2020 and the most recent shelter in place drill was conducted 06/05/2019. Standard #: 22VAC40-185-160-A
Description: Based on review of eight staff records it was determined that not all records contained documentation of a TB (tuberculosis) screening within the first 21 days of employment. Staff A (start date: 01/29/2020) did not have documentation of a TB screening on file.

Plan of Correction: Administration will obtain a current TB screening from staff A.

Standard #: 22VAC40-185-160-C
Description: Based on review of eight staff records it was determined that not all records contained documentation of an updated TB (tuberculosis) screening every two years. Evidence: The most recent TB screening on file for Staff B is dated 12/12/2017.

Plan of Correction: Administration will obtain an updated TB screening from Staff B.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of eight staff records it was determined that not all records contained documentation of the findings of a central registry background check within the first 30 days of employment. Evidence: Staff A (start date: 01/29/2020) and Staff C (start date: 10/14/2019) did not have documentation of a central registry check on file.

Plan of Correction: Administration will inquire the status of the central registry checks for Staff A and Staff C.
