Comments:
A Subsidy Health and Safety inspection (SHSI) was conducted on Friday, December 20, 2019 to review supplemental health and safety requirements. The inspector arrived at 9:23 AM and concluded the inspection at 2:35 PM. The census for today's inspection consisted of 41 children. During the inspection, children were playing with musical instruments, reviewing colors and shapes, coloring, working puzzles, reading, getting diapers changes, bathroom breaks, washing hands, eating lunch and napping. Medication is administered and medication, authorization forms and allergy plans were reviewed. Children's equipment, learning materials, indoor and outdoor space, restrooms, first aid kit and the nonmedical emergency supplies were reviewed. Five children's records and five staff records, annual fire and health inspections, fire drill, shelter in place drill and lockdown drill logs, daily attendance and all required postings were reviewed.

The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-665-540-A
Description: Based on a record review, the provider did not ensure that all staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment, within the last 30 calendar days of the date of hire and prior to coming into contact with children. The documentation shall be signed by a physician, physician's designee, or an official of the local health department.

Evidence:
1. The record for staff #2 (date of hire 01/30/2019) had a completed TB screening dated 04/07/2019.
2. The record for staff #3 (date of hire 10/15/2019) had a completed TB screening dated 12/12/2019.
3. The record for staff #5 (date of hire 09/30/2019) had a completed TB screening dated 11/16/2019.
