Comments:
An unannounced monitoring inspection was conducted on Friday, December 20, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 9:23 AM and concluded the inspection at 2:35 PM. The census for today's inspection consisted of 41 children. During the inspection, children were playing with musical instruments, reviewing colors and shapes, coloring, working puzzles, reading, getting diapers changes, bathroom breaks, washing hands, eating lunch and napping. Children's equipment, learning materials, indoor and outdoor space, restrooms, first aid kit and the nonmedical emergency supplies were reviewed. Medication is administered and medication, authorization forms and allergy plans were reviewed. Five children's records and five staff records, annual fire and health inspections, fire drill, shelter in place drill and lockdown drill logs, daily attendance and all required postings were reviewed.

The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. On the playground, a toddler climber had soft vinyl blocks on the floor of the climber. The vinyl was torn in five different areas with exposed foam. Children have access to the toddler climber.
2. The director stated that the equipment is considered broken.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In classroom #1, one aerosol spray can of glass cleaner labeled "warning" and "Hazardous" was on the bottom shelf of an unlocked cabinet. Children in this classroom are under two years of age.
2. In classroom #4, one can of Lysol disinfectant spray labeled "Danger," "Hazards to humans" and "Keep out of reach of children" was on the bottom shelf in an unlocked cabinet. Children in the classroom are three years of age.

Description: Based on a review of staff records, the center did not obtain satisfactory results of a fingerprint-based national criminal background check prior to the first day of employment.

Evidence:
The record for staff #1 (date of hire 01/02/2019) had fingerprint results dated 01/24/2019.
