Comments:
An unannounced monitoring inspection took place on March 10, 2020. There were 83 children in care from ages three months old to three years old with 23 staff providing care and supervision. The inspector reviewed a sample of eight children's records and eight staff records during the inspection. The LI also reviewed a sample of four medications. Per staff, this center does not transport children. Children were observed during the following activities: activity stations, infant feeding, cleaning and sanitizing, free play, story time, and lunch. The inspector also reviewed emergency drill documentation, first aid kits, the physical plant, and emergency maps. There were discussions about diaper changing stations and infant daily reports. The inspection took place from approximately 9:20 AM to 2:00 PM. Violations were discussed at the close of the inspection. Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:

1. Located on the playground, there was a broken plastic cabinet, with a detached and cracked door, on the ground.
2. Also located on the playground, the LI observed a trashcan resting on the fence. The trashcan had a broken and jagged outer edge.

Plan of Correction: The staff member will remove those items today.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:

1. A bottle labeled "bleach and water" and a bottle labeled "soap and water" were both located in the "Lilac Room" in an unlocked upper cabinet, in the children's bathroom.

Plan of Correction: We will remind staff to lock the cabinet when those items are not in use.

Standard #: 22VAC40-185-560-G
Description: Based on observations and discussion with staff, the center failed to ensure that when food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.

Evidence:

1. The LI observed one lunchbox in the hallway and one lunchbox in the "Sunflower Room" without the child's identification.
2. Staff confirmed that at least one of the lunchboxes was not labeled in such a way that identified that owner.

Plan of Correction: Staff will check that parents have adhered to the Parent Handbook policy, to label lunchboxes with their child's name.
