Comments:
An unannounced monitoring inspection was conducted from 7:20 - 8:30 am with staff. There were 21 children, ranging from five years to 11 years of age, present with two staff supervising. Children were observed playing in centers that included arts and crafts, doll house, hot wheels cars and playing a group game. Five children's records were reviewed and in compliance. Two on-site staff records were reviewed. Four medications and authorization forms were reviewed. The last emergency escape drill was conducted: 2/5/20 and the last shelter-in-place drill was conducted: 2/14/20. Description: Based on a review of on site documentation, it was determined that not all required information was available for all staff. Evidence: Staff #1 and staff #2 did not have documentation of the address for an emergency contact to be notified in an emergency.

Standard #: 22VAC40-185-350-E-5
Description: Based on observation, the facility failed to maintain the required ratio of one staff for every 18 children, ages five years to eight years of age. Evidence: 1. One staff member left the group in the cafeteria to open the door (for arriving teacher/children), leaving one staff alone with 20 school age children for approximately two minutes. The staff left a second time leaving the staff alone with 25 children. The youngest child in attendance today is five years old.

Description: Based on review of four medications, the facility failed to get written parental authorization for each medication on site. Evidence: child #1 did not have a long term authorization form signed by a parent and physician; child #2 did not have a long term authorization form signed by a parent; child #3 did not have a long term authorization form signed by a parent.
