Comments:
A Subsidy Health and Safety inspection was conducted on 3/11/209 from 3:30pm - 5:00pm. During the inspection there were 39 children ages five to 10 years old in care with three staff. Children were observed participating in various activities in the cafeteria, playing in the gym, eating dinner, and doing homework. Records were reviewed for five children while at the facility. The records for five staff were reviewed on 3/12/20 at the central office. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on interviews and observation, it was determined that the vendor did not ensure to post, with parental approval, or keep in a way that is immediately accessible to staff, a current list of all children?s allergies, sensitivities, and dietary restrictions.

Evidence:
1. There was not a current list of all children?s allergies, sensitivities and dietary restrictions for the children enrolled at the facility.
2. Staff #1 confirmed that there was not a current list of all children?s allergies, sensitivities, and dietary restrictions for the children enrolled at the facility did not include all of the children.

Plan of Correction: The vendor responded: Site Director has gone through all files and has created an allergy list and posted it in cabinet.

Standard #: 22VAC40-665-510-B
Description: Based on observation and interviews, it was determined that the vendor did not ensure that for each group of children, the vendor shall maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
1. The Licensing Inspector reviewed the attendance records for the facility, and the attendance records did not have the departure documented for all children present on multiple days.
2. Staff #1 confirmed that there was not a written record of daily attendance that documented the arrival and departure of each child in care as it occurs.

Plan of Correction: The vendor responded: Site Director will now ensure the attendance roster is completed daily acknowledging the arrival and departure of each child.
