An unannounced monitoring inspection was conducted on 03/13/2020 from 2:50 pm - 4:30 pm. At the time of entrance there were 47 school age children in care with three staff. Children were observed during outdoor play, doing homework and playing games in the cafeteria. An afternoon snack was served at approximately 3:30 pm.
Records were reviewed for eight children.
Areas of noncompliance are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.

Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that children's records contain all of the required elements.
Evidence:
1. The enrollment record for child 1 lacked a work phone number for each parent's place of employment.
a. The record indicated that each parent was employed.
2. The enrollment record for child 2 lacked an address for the second emergency contact for child 2.Plan of Correction: The parent's of child 1 and child 2 will be asked to provide missing information.
Standard #: 22VAC40-185-340-F Description: Based on observation, the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence:
1. Two children, (child 3 and child 4 who are both eight years of age) , were not within sight supervision of staff 1 when they walked from the outdoor play area to the cafeteria which is located at the opposite end of the school from the playground.
a. Staff 1 did stand in the lobby area of the doorway however he could not see the children from this area once they entered into the hallway.Plan of Correction: A system will be put in place to ensure children are always within sight and sound supervision of staff when the walk from the playground toward the cafeteria.
Standard #: 22VAC40-185-430-K Description: Based on observation, the center failed to make provision for an individual place for each child's belongings.
Evidence:
A total of 9 backpacks, 3 coats, 1 hoodie and a set of markers were observed strewn about on the floor up against the wall in the cafeteria.Plan of Correction: Going forward, children will be monitored to ensure their belongings are placed in the baskets available as the individual storage place.
Standard #: 22VAC40-185-510-E Description: Based on observation, the center failed to ensure that medication shall be labeled with the child's name.
Evidence:
One bottle of over-the-counter medication was not labeled with the child's name.Plan of Correction: Medication was labeled during the inspection.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation, the center failed to post the findings of the most recent inspection of the facility.
Evidence:
The findings from the most recent inspection, conducted on 11/14/2019, were not posted.Plan of Correction: The acknowledge form for today's inspection will be posted. Once the inspection reports are received these will be posted.
