Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted on March 11, 2020. Upon entrance there were 23 children and 4 staff present with children. There were additional staff on site for administrative and program purposes. The record review sample consisted of 5 children's records and 3 staff records.
A licensing inspection was conducted on the same date as the Subsidy Health Safety Inspection (SHSI) supplemental inspection; the two inspections combined cover the Subsidy Health and Safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on review of five children's records and staff interviews, the vendor did not ensure there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The records for child #5 did not have an allergy action plan. It was noted on the physical from the physician that child #5 has an allergy to peanut.

Plan of Correction: The Center Director will get the plan from the doctor.

Standard #: 22VAC40-665-580-B
Description: Based on review of three staff records and staff interviews, the vendor did not ensure each staff had within the first 90 days of employment or subsidy vendor approval all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff.

Evidence: The records for staff #2, date of hire 9-10-2019 and staff #3, date of hire 9-26-2019 did not include documentation of Preservice training.

Plan of Correction: The Center Director will ensure the staff obtain the training.

Standard #: 22VAC40-665-650-E
Description: Based on interviews and documentation review, the vendor did not ensure to develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members.

Evidence: The Center Director confirmed there was not a written plan to address consistent care by staff.

Plan of Correction: The Center Director will update consistent care by staff in employee handbooks.
