An unannounced complaint inspection was conducted from 7:00 - 10:00 am to investigate an allegation of Supervision of Children - Ratios, Procedures for Emergencies, and Behavioral Guidance. Observations were conducted, staff were interviewed and documentation was reviewed. Based on evidence gathered, the allegation was not substantiated. The violation found was not complaint related. Standard #: 22VAC40-185-340-F Complaint related: No Description: Based on observation, it was determined that the facility failed to ensure that children under 10 years of age are always within actual sight and sound supervision of staff. Evidence: school age children were observed leaving the classroom to go into the hall and get their belongs from lockers. There was no teacher in the hallway or positioned in such a way where they could see and hear all the children. Plan of Correction: Staff member was near doorway and could see the lockers and children thru the open door and window as they gathered their belongings for school. Immediate corrective action was taken, staff member placed herself in the hallway while children went to their lockers and will continue to do so.
