Comments:
An unannounced renewal inspection was conducted on site on March 6, 2020 from 3:45 pm - 5:00 pm. There were 12 children in care ages five to 10 years old with two teachers. Children's records and staff emergency contact information were reviewed on site. Staff records were reviewed at the central location on March 2, 2020. A walking of the center inside was completed. Required postings, annual inspections, emergency evacuation drills first aid kit and medication were reviewed. Children were observed in teacher and self-directed activities including transitions, restroom routines, hand washing, snack, and craft activities. Violations regarding ratio and background checks were identified and appear on the violation notice. A single violation was identified during record review and appears on the violation notice.

Description: Based on record review the center failed to ensure that each staff record contains all required information.
Evidence:
The record for staff 1 does not contain documentation of information to demonstrate that staff 1 possesses the education required by the job position.

Plan of Correction: Program Director emailed staff one to obtain education documentation required for the position. Email was sent on 3/2/2020. The Program Director and Childcare Administrator will conduct staff records audits quarterly to obtain the required information and make sure it is maintained in each staff record. The program administrator and director(s) will be responsible for these actions. Correction date of 3/8/2020.
