Comments:
A complaint investigation was initiated on March 8, 2020 and ended on March 10, 2020 regarding noncompliance with classroom ratios. Interviews were conducted and documentation was reviewed. There was a preponderance of evidence to support the allegation regarding ratios and the complaint is deemed valid. The complaint related violation and one non-related violation are identified in the Violation Notice.

Description: Based on record review and staff interview, 4 out of 5 days, the center did not maintain a ratio of one staff member for every four children that were in care, for children from birth to the age of 16 months.

Evidence:
1. Based on a review of the child attendance and transition sheets for March 2-6, 2020, 4 days documented that the Infant 4 classroom (children ages 6 weeks-15 months) was over the required ratio.
2. Based on a review of the child attendance and transition sheets for March 2-6, 2020, March 5, 2020 documented that the Infant 3 classroom (children ages 6 weeks-15 months) had five children and one staff member from approximately 9:40am-10:00am.
3. During staff interview, Staff B confirmed that the Infant classrooms are out of ratio.

Plan of Correction: Additional training and support for teachers will be provided on tracking attendance and transitions. In addition, staff will be required to notify leadership staff within one minute of reaching the classroom's maximum limit per ratio standards.

Description: Based on observation, when food is brought from home, the center did not ensure that food containers are sealed and clearly dated and labeled in a way that identifies the owner.

Evidence: In the Preschool classroom (Room 209; children ages 3-4 years old), 2 of 3 children's food containers brought from home were not labeled with the child's name and date.

Plan of Correction: Leadership staff will provide additional training and support when food is brought from home to ensure that food containers are sealed, clearly labeled with date and name, that identifies owner.
