Comments:
A complaint investigation was initiated on March 10, 2020 with another local agency and ended on March 19, 2020 regarding noncompliance with classroom ratios. Interviews were conducted and documentation was reviewed. There was a preponderance of evidence to support the allegation regarding ratios and the complaint was deemed valid. The complaint related violations and non-related violations are identified in the Violation Notice.

Complaint related: No
Description: Based on record review, a volunteer did not submit documentation of a negative tuberculosis screening.

Evidence: Volunteer C (start date: 2019) did not have documentation of a negative TB screening.

Plan of Correction: It is not our practice to background check parents in Head Start. We will ensure that all parents. We will no longer use volunteers in the adult-child ratio.

Description: Based on record review, volunteers did not receive the following training by the end of their first day of assuming job responsibilities.

Evidence: Volunteers A, B, and C are counted in ratio and do not have documentation of completed orientation training.

Complaint related: Yes
Description: Based on staff interview, for children from three years to the age of eligibility to attend public school, five years by September 30, the center did not maintain one staff member for every 10 children.

Evidence:
1. Staff A stated, "We are over ratio because we don't have enough staff. Sometimes, 2 staff with 22 or 23 children."
2. Staff B stated, "There was a time when me and my co-teacher has more than 20 children; maybe 22".
3. Volunteer B stated, "I have seen the classroom have more than 10 children with 1 staff".

Plan of Correction: We will take a daily count of children and assign staff as needed to cover the appropriate ratio. We will also use the Name to Face tracking form hourly to document the ratio for the particular classroom.

Description: Based on record review, the center did not ensure that a volunteer of a licensed child welfare agency must not provide volunteer service until the agency has the person's completed sworn statement or affirmation.

Evidence: Volunteer C (start date: 2019) did not have a completed sworn statement or affirmation.
