An unannounced complaint investigation was conducted on 2/25/2020 in response to multiple complaints received by the licensing office on 01/30/2020, 02/13/2020, and 02/25/2020. On 02/25/2020 inspector investigated allegations regarding ratios and water supply. On 02/27/2020 inspector investigated staff training, ratios, behavioral guidance, transportation, and water supply. The information gathered during the investigation does support the allegations regarding ratio and behavioral guidance, so the complaint is determined to be "valid". Standard #: 22VAC40-185-350-E-5 Complaint related: Yes Description: Based on interviews it was determined the center did not ensure that the proper ratios were maintained at all times.
Evidence:
1. A staff member reported that there have been issues with ratios "about 6 months ago in the mornings due to staff late arrival".

Standard #: 22VAC40-185-400-A Complaint related: Yes Description: Based on interview it was determined the center did not ensure that all staff interactions with children in care provided the needed help, comfort, and support.
Evidence:
1. A staff member reported that the director has had to address staff's voice tone with the children " a few times".
2. A staff member reported that another staff member "screams, is loud and has been overheard by parents being harsh".
3. A staff member stated that one staff member is "really loud and abrupt at times".
