Comments:
The licensing inspector completed an unannounced complaint inspection on February 13, 2020 in response to a complaint that was received by the Department on January 14, 2020 concerning the care and protection of children during diapering procedures. The investigation was initiated on January 14, 2020 and completed on April 7, 2020. On February 13, 2020, two inspectors were on site from 9:25am to approximately 11:10am interviewing staff, conducting observations at the center, and reviewing documentation. The preponderance of the evidence gathered during the investigation supports the allegation regarding the care and protection of children, therefore the complaint is determined to be valid. Additional violations unrelated to the complaint were found and are identified on the violation notice. See the violation notice for more details.

Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Complaint related: Yes
Description: Based on interviews, the center did not ensure when staff are supervising children, they shall always ensure their care, protection, and guidance.

Evidence: During interviews conducted on 01/14/20 and 02/13/20, the following was revealed:
1) Witness #1 reported that on 01/13/20, Child #1 (2-year-old) was observed after Staff #1 changed his diaper and stains that appeared to be bleach stains were observed on the back of his pants.
2) Administrator #1 reported that it was determined by the center that the stains on Child #1?s pants were caused by the bleach/water solution that Staff #1 sprayed on the diapering surface on 01/13/20.
3) Staff #1 and Staff #2 reported that in the past, they thought the bleach/water solution used to sanitize the diapering area and tables was not diluted correctly and had a ?strong odor? of bleach. Staff #1 and Staff #2 reported the bleach/water solution was still used by staff to sanitize.
4) Staff #1 did not ensure Child #1's care and protection by not ensuring the bleach/water solution used to sanitize the diapering surface was fully dried before placing another child on it.

Plan of Correction: Per the Center: "I spoke to my cook - the person responsible for fixing the bleach/water solution and reminded her to be more accurate when mixing the bleach/water solution. We went over the right portions together."

Complaint related: No
Description: Based on observations on 02/13/2020, the center did not ensure that staff shall wash their hands with soap and running water before and after helping a child use the toilet or a diaper change, after the staff member uses the toilet, after any contact with body fluids, and before feeding or helping children with feeding.

Evidence: 1) During observation in the Twos classroom, the inspector observed Staff #1 changing two children's diapers consecutively. Staff #1 was not observed washing their hands before or after each diaper change. The staff had on a pair of gloves when diapering both children.

Plan of Correction: Per the Center: "During our next monthly staff meeting - all staff were trained on the proper way to change a diaper."

Complaint related: No
Description: Based on observation and interview on 02/13/2020, the center did not ensure the diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use.

Evidence: 1) During observations in the Twos classroom, the licensing inspector observed Staff #1 changing two children's diapers consecutively. Between each diaper change, Staff #1 was observed spraying the diapering surface and immediately drying it with a paper towel. 2) During interview, Staff #1 reported the contents in the bottle contained only soap and water. Staff #1 acknowledged the bottle with disinfectant solution was not used between diaper changes while the inspector was conducting observations.

Plan of Correction: Per the Center: "During our monthly staff meeting, all staff were retrained on how to properly clean and sanitize the diaper changing area."
