A complaint was received on October 21, 2019 and initiated on October 23, 2019. The investigation was concluded on December 13, 2019 during an onsite inspection to investigate the allegations of lack of supervision, lack of documenting injuries. Staff and children's records were reviewed. Interviews were conducted in person and by telephone. Injury reports were reviewed. The information gathered during the investigation does not support allegation. Therefore, the complaint is not valid. there were not any other violations identified during the investigation.
