The Licensing Inspector conducted a joint investigation with two local agencies in response to a self reported incident that occurred on 2/21/2020. Interviews were conducted with staff, written statements from 5 staff members, an accident report, medical documentation and staff records were reviewed. The information gathered during this investigation does not support the allegations being cited regarding the self report; however other violations were cited on the Violation Notice and were discussed with the center director. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 5 calendar days of receipt. Standard #: 22VAC40-185-70-A Description: Based on record review, the center did not maintain all required information for staff records.
Evidence:
1. Staff #1's (date of hire 9/18/2019) file was missing one reference.Plan of Correction: Missing reference has been located and placed in staff's file as of 2/25/2020.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, the center did not have the results of a central registry check within 30 days of employment of a staff member.
Evidence:
1. Staff #4 began employment on 12/18/2019. The center did not have the results of the central registry that was sent on 1/05/2019 during the inspection. The center did not have documentation of following up for the results within 30 days.Plan of Correction: Current background check received on 3/18/2020 up to date background check is on file at center. Central registry results received 1/27/2020. Current central registry is on file at center.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review, the center did not obtain the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #2 (Date of hire 8/27/2019) lived in MD in the last 5 years and Staff #4 (Date of hire 12/18/2019) lived in MD in the last 5 years and neither staff member had the Out of State Central Registry results in their files and did not have documentation that the forms had been sent during the inspection.Plan of Correction: Current background check received on 2/26/2020. Up to date background check is on file at center. Central Registry results received on 9/19/2019. Current central registry is on file at center.
