This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on Wednesday, May 6, 2020 and concluded on Thursday, May 7, 2020. The director was contacted by telephone to initiate the inspection.There were 16 children present and 6 staff. The inspector emailed the director list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 22VAC40-185-140-A Description: Based on review of children's record and documentation, the provider did not ensure to obtain a physical for all children within one month after attendance.
Evidence: In Child#1 record (start date: 8/09/19), there was no physical documentation found during the inspection on 05/07/2020. The documentation from center's owner confirmed Child#1's physical was not found for review during the inspection.Plan of Correction: Due to review of Child#1 record, it was determined that document was not satisfied. A copy of Child#1 physical was requested and obtained by center today to include in the record.
