Comments:
A joint investigation was conducted with local CPS and law enforcement on February 5, 2020 and with CPS on 02/07/2020. The inspection was in response to a self-reported incident received by the Valley Licensing Office on January 27, 2020, in regards to the licensing standards relating to the care and protection of children in care. Areas of non-compliance may be found on the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to
me within five (5) business days from today. You will need to specify how the deficient practice will be or has been corrected. Just
writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the
standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each
step and/or monitoring any preventive measure(s).

Violations:
Standard #: 22VAC40-185-340-A
Description: Based on staff interviews, the center failed to ensure the care, protection and guidance of one child in care.

Evidence:
1. Staff #1 stated on January 27, 2020, she had picked up 15 children from school to transport back to the center.
2. Staff #1 stated she arrived at the center at approximately 4:00 pm.
3. Staff #2 was waiting outside for the arriving students.
4. Staff #1 stated the children exited the van and Staff #2 assisted them inside the building.
5. Staff #1 stated she pulled the van forward to a parking spot and went inside the center because she was ill.
6. Staff #1 stated she did not verify the van was empty before going inside.
7. At approximately 5:32 p.m., Parent #1 one arrived to pick up Child #1.
8. Parent #1 entered the classroom and Staff #3 stated Child #1 was not present.
9. Center staff called Staff #1 to determine if of Child #1 had been picked up from school to be transported back to the center.
10. Staff #1 confirmed Child #1 was picked up from the school.
11. Staff #4 and Parent #1 went outside to look in the van.
12. Child #1 was found at approximately 5:38 p.m. in the van unsupervised and crying.

Plan of Correction: "Cameras were off 15 minutes. Staff #1 arrived at 4:15 p.m. Staff #1 asked Staff #2 if we were good. Staff #2 appeared to check the van and said yes. Staff #1 backed into a parking lot."

1. New checklist logs for staff dropping off and the staff waiting outside.
2. Staff in classrooms are to check with management about any absent children.
3. Cameras have been installed in the vans.

Standard #: 22VAC40-185-340-F
Description: Based on staff interviews, the center failed to ensure one child under the age of 10 was in actual sight and sound of staff.

Evidence:
1. On January 27, 2019, at approximately 4:00 p.m., the center van arrived with fifteen children.
2. Staff #2 was waiting outside for the arriving students.
3. Staff #1 stated the children exited the van and Staff #2 assisted them inside the building.
4. Staff did not count the number of children that exited the vehicle.
5. At approximately 5:38 p.m. Child #1 was found unsupervised on the center van.
6. Child #1 was out of actual site and sound supervision from approximately 4:00 p.m. until 5:38 p.m.

Plan of Correction: "Timing is wrong."

Standard #: 22VAC40-185-580-C
Description: Based on staff interviews, the center failed to ensure child restraints were used for three children being transported.

Evidence:
1. On January 27, 2020, fifteen children were picked up from school to be transported to the center.
2. Three children on the van were 4 -years of age and were not placed in a child restraint seat.
3. Staff #1 confirmed three children, 4- years of age, were not in a booster seat.

Plan of Correction: "Disagree. There are booster seats available in all vans. It was never confirmed by Staff #1 that children were not in booster seats."

"No plan of correction there are booster seats in all the vans."

Standard #: 22VAC40-185-580-K
Description: Based on staff interviews, the center failed to verify that all children were removed from the vehicle at the conclusion of a trip.

Evidence:
1. Staff #1 stated on January 27, 2020, she had picked up 15 children from school to transport back to the center.
2. Staff #1 stated she arrived at the center at approximately 4:00 pm.
3. Staff #2 was waiting outside for the arriving students.
4. Staff #1 stated she did not provide Staff #2 with the number of children on the van.
5. Staff #2 confirmed Staff #1 did not provide the number of children that were on the van.
6. Staff #1 stated the children exited the van and Staff #2 assisted them inside the building.
7. Staff #1 stated she pulled the van forward to a parking spot and went inside the center because she was ill.
8. Staff #1 stated she did not verify the van was empty before going inside.

Plan of Correction: "Staff #2 claimed she checked the van."
