Standard #: 22VAC40-185-140-A Description: Based on a review of four children's records and interview, the center did not ensure that one child had a physical examination by or under the direction of a physician before the child's attendance; or within one month after attendance.
Evidence: 1) The physical examination in the record for Child #4, enrolled on 06/17/2019, was dated 05/14/2020. 2) During interview, a member of management confirmed there was no physical examination on file for Child #4 prior to 05/14/2020, which was received by the center on 05/28/2020.Plan of Correction: The physical examination was requested from the parent and submitted on 05/28/2020.
