This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A Renewal inspection was initiated on 6/16/2020 and concluded on 6/19/2020. The director was contacted by telephone to initiate the inspection. There was 36 children present and 7 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 6 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-140-A Description: Based on children records review, 2 out 6 children records, the center did not maintain a physical examination within one month after child's attendance.
Evidence: 1. Child #1 (date of attendance 8/28/2017) did not have a physical examination available for review. 2. Child #2 (date of attendance 6/28/2015) did not have a physical examination available for review.
