Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted on March 12, 2020. The on-site inspection was from 12:11 p.m. to 3:20 p.m. On this date, there were 94 children present in seven classrooms and in the care of fourteen staff members. Three administration members were available to assist if needed. The total enrollment for the program was 140 children. The exceptions to compliance were noted on the violation notice and the risks ratings were noted on the supplemental page. There were five staff members' records and nine children's records reviewed. Seven classrooms were toured and observations were made in three of those rooms. Eight staff members and six children were interviewed. No medication is being administered. All interactions between children and staff were age appropriate and positive. Some items that were reviewed and observed included: required posted information, injury reports, documentation of evacuation, shelter-in-place, and lockdown drills, emergency supplies, first aid kit, warm water temperature, updated emergency preparedness plan and injury prevention plan, asbestos management plan, outdoor play, transitions, circle time, singing, infant care, lunch, and nap time.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Based on documentation review, the center failed to ensure the care and protection of children.

Evidence: The center did not obtain an asbestos reinspection every three years as required by the asbestos management plan. The last inspection was completed 8/26/16.

Description: Based on review of five injury reports, program did not include all required information on the form.

Evidence: 1. Form did not include future action to prevent recurrence of the injury. 2. Program Administrator confirmed she did not realize this requirement existed.
