This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on Wednesday, June 17, 2020 and concluded on Thursday, June 18, 2020. The director was contacted by telephone to initiate the inspection. There were 16 children present and 4 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. As of 6/30/2020, corrective actions of the violations have not been provided. Standard #: 22VAC40-185-60-A Description: Based on review of children's record and interview with staff, the center did not ensure to obtain all information for children's record.
Evidence:In Child#1's record (start date:12/09/19), the name, address, number for the second contact person to call in emergency if a parent cannot be reach was not available. Standard #: 22VAC40-191-40-D-6 Description: Based on review of staff records, documentation, and interview with staff, the center did obtained a required criminal history record report or a central registry finding from an applicant dated more than 90 days prior to the date of employment.
Evidence:
1. In Staff#2's record (start date:11/01/2019) the Office of Background Investigation (OBI) determination letter was dated 06/18/19.
2. Staff#2's employment application form documentation revealed start date as 11/01/19.

Evidence:1. In Staff#1's record, the date of hire was documented 11/25/19 on the employment application form and the OBI determination letter was dated 2/19/2020
2. A review of Staff#1's orientation certificate completed by the center was dated 12/04/19.
