Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A focused inspection was initiated on 07/14/2020 and concluded on 07/14/2020. The provider was contacted by telephone to initiate the inspection. There were 2 children present and 4 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with an applicable standard or law and a violation was documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not ensure two of two children's records contained all of the required information.
Evidence:
1. The record of child #1 (start date: 6/15/2020) did not contain documentation of the parents employment, employment number, an address for two emergency contact persons.
2. The record of child #2 (start date: 7/8/2020) did not contain documentation of the parents employment, employment number, an address for two emergency contact persons.

Plan of Correction: In the future we will be implementing the state approved child registration form. I will have parents correct form upon pickup.

Standard #: 22VAC40-185-90--A
Description: Based on record review, the center did not ensure two of two children's records contained documentation of a written agreement between the parent and the center authorizing and agreeing to the following: emergency medical care, a statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible, and a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease.
Evidence:
1. The record of child #1 (start date: 6/15/20) did not contain documentation of written agreements between the parent and the center as required by 22 VAC 40-185-90.
2. The record of child #2 (start date: 7/8/20) did not contain documentation of written agreements between the parent and the center as required by 22 VAC 40-185-90.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, the center did not ensure one of two staff records contained documentation of an Out of State Child Abuse and Neglect Search Request by the End of 30th Day of Employment.
Evidence:
The record of staff #1 (start date: 5/18/2020) did not have documentation of an Out of State Child Abuse and Neglect Search Request. The record of staff #1 indicated that the staff has lived out of the state of Virginia within the last 5 years.

Plan of Correction: In the future out of state central registry checks will be done upon hire. I requested up to date registry information.
