The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
- Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring/ inspection was initiated on 7/16/2020 and concluded on 7/17/2020. The director was contacted by telephone to initiate the inspection. There were 34 children present and 8 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 63.2(17)-1720.1-B-3 Description: Based on a review of staff record documentation the program did not obtain an Out of State Child Abuse and Neglect Search Request by the End of 30th Day of Employment for all employees who resided outside of Virginia within the past 5 years. Evidence: The inspector requested to review the Out of State child abuse and neglect registry search for staff #1 who indicated on her Sworn Disclosure that she resided in North Carolina within the past 5 years. The Sworn Disclosure form was completed and signed on 5/3/18. Staff #1 began employment at the center 9/1/08 and is currently still working in the program. The program director stated that she was unable to locate this documentation, though she recalled having requested it. Plan of Correction: The program director stated that she contacted the NC agency to attempt to obtain a copy of the NC Registry check but was unable to fax the required form to obtain it. She stated that she will continue to fax the required form but also had Staff #1 complete a new request for the NC Registry check which was sent by certified mail. The results will be filed in Staff #1's record upon receipt and a copy of the new request will be forwarded to the licensing inspector within 10 days.
