Technical Assistance:
Staff should be retrained on Daily Heath Observation and review the requirements of the standards of supervision.

Inspector also discussed daily operation practices that ensure the health, safety and proper guidance of children in care.

Comments:
An investigation was initiated on July 6, 2020 in response to a self-reported incident received by our office regarding the care and protection of a school aged child. Based upon multiple interviews, the preponderance of evidence does support a violation;indicated in the violation notice . Due to COVID 19 this investigation was not conducted on site.

Violations:
Standard #: 22VAC40-185-340-A
Description: Based on interview with staff it was determined that staff responsible for supervising children did not ensure their care, protection, and guidance.

Evidence:1. During interviews with multiple staff- no supervising staff was able to indicate when and how the school age child in care fell and hurt their arm or when or why shoes were not worn during the time of the incident.
2. When asked about the procedures for handing out snack inspector was unable to determine how proper guidance of children was maintained to insure safety; Staff 1 said that children are to be seated Staff 2 stated that Children are lined up and can later get 2nd helpings freely. Staff 3 stated that they "did not know" .
3. During interviews with staff it was determined that supervising staff did not properly assess the injury of a child at the time the injury occurred. 2 of 4 staff stated the severity of the injury was not determined until the director assessed the injury after the supervising staff applied first aid.
