Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 8/11/20 and concluded on 8/12/20. The director was contacted by telephone to initiate the inspection. There were 65 children present and 16 staff present. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 14 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:

Standard #:	22VAC40-185-60-A
Description:	Based on a review of four children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information. Evidence 1. The record for child #1 did not contain the addresses for the emergency contacts or the employment information for either parents. 2. The record for child #2 did not contain the addresses for the emergency contacts or the employment information for either parents. 3. The record for child #3 did not contain the addresses for the emergency contacts or the employment information for either parents. 4. The record for child #4 did not contain the addresses for the emergency contacts or the employment information for either parents. 5. Staff #15 (Program Director) confirmed that none of the records reviewed contained all of the required information.
Plan of Correction:	The facility responded: We will add the items missing from on enrollment form so that it contains all of the required items. We will have the parents of all the children add the missing information to their child's records.
Standard #:	63.2(17)-1720.1-B-2
Description:	Based on a review of 14 staff records, it was determined that the facility did not ensure that an employee is allowed to begin employment without a completed national criminal history record check (finger printing). Evidence: 1. The record for staff #1 (date of hire 7/15/20) contained documentation of a completed national criminal history record check (finger printing) that was dated 8/7/20, which was indicates it was completed after staff #1 began employment. 2. Staff #15 (Program Director) reviewed the record for staff #1 and confirmed that new employees were allowed to began employment without a completed national criminal history record check (finger printing).
Plan of Correction:	The facility responded: In the future, all newly hired staff will complete a national criminal history record check (finger printing) before beginning employment.
