An announced monitoring inspection was conducted at the request of the licnesee for transfer of location from the Thomas Hunter Middle school to the Mathews Boys Girls Club (498 Church Street, Mathews VA 23601). The licensing inspector was on site and observed the physical plant of the new location on August 18, 2020. Additional items required for the inspection were conducted conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on August 18, 2020 and concluded on August 19, 2020. The inspector emailed the director a list of items required to complete the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not ensure that the areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
Evidence: The following were observed in the building at 498 Church Street, Mathews VA 23109.
1) The toilet seats were stained and discolored in both the boys and girls restroom
2) One toilet bowl in the boys restroom has a rust colored stain.
3) The white plastic covering on the walls in the gymnasium was peeling off exposing a tan colored material.
4) The white plastic covering on the ceiling in the multipurpose room was peeling off and exposing ceiling insulation. The insulation was not accessible to children.Plan of Correction: Temporary repairs of the multipurpose room ceiling, and boy's bathroom toilet will be completed by 8/30/2020. Permanent repairs for the ceiling, toilet , toilet seat covers, will be completed 10/15/2020. Phase 1 temporary repairs will be made to the ceiling in the multipurpose room to put the visible insulation back in place. A newer toilet will be set in place of the rusted toilet in the boy's bathroom work will be completed by August 30, 2020. Phase 2 permanent repairs will be completed by the Count Administrator and Building Official who have released funding for this project. Funding will be used to obtain licensed contractors to perform the necessary work to repair the white plastic coverings on the ceiling of the multipurpose room,replace one toilet in boy's bathroom and replace all seats in the bathrooms. County work will be completed on October 15, 2020. At this time, the gymnasium will be off limits until repairs can be made.
