Technical Assistance:
1. Child records should identify each parent, or "N/A" if there is not a second parent. Spaces for this information on the floor should not be left blank. 2. Reviewed/trained vendor on new subsidy requirements. 3. Forms provided: attendance, and new emergency plan.

Comments:
10:50am-12:15pm Annual unannounced subsidy inspection completed. Observed 3 children = 6 points with the vendor. Ratios and supervsion were in compliance. Children were having active play outside on a playground adjacent to the home, playing group games, having a water break, and playing blocks inside. The vendor was talking and playing with the children. The home was found to be clean, safeguarded appropriately, and sufficiently supplied with toys and equipment for the children. The vendor participates in the USDA Food Program, and is currently certified in CPR/First Aid. One recordkeeping non-compliance found.

Description: Based on records reviewed, it was determined that 1 of 4 children's records did not include required information. Evidence: Child #5-Written authorization for emergency medical care.

Plan of Correction: The vendor said she will ask the parent to complete and sign this part of the form today.
