Technical Assistance:
Technical assistance was provided regarding staff emergency contacts kept at the center. The emergency contacts shall include the name, address and telephone number of a person to be notified in an emergency for each staff. In addition, technical assistance was also provided regarding acceptable forms of documentation of tuberculosis screening. Documentation shall include at least one of the following: 1) A clearance statement signed by a physician, the physician's designee or an official of the local health department, 2) The results of a negative tuberculin skin test (TST). The documentation shall include the date the test was given and the results of the test and be signed by a physician, physician's designee or an official of the health department, or 3) The results of a chest x-ray negative for active tuberculosis disease.

Comments:
An announced non-mandated, focused inspection was conducted on 1/16/20 between the hours of 1:30pm to 4:30pm at the Massad Family YMCA located in Falmouth, Virginia. A total of five staff files were reviewed. Please email Lori.Hancock@dss.virginia.gov with any questions.

Description: Based on record review, 1 of 5 staff records reviewed did not contain documentation that an out of state central registry search request was sent for all staff who have resided outside the state of Virginia within the last 5 years. Evidence: staff A (date of hire: 11/1/18) has resided outside the state of Virginia within the last 5 years and there was no documentation an out of state central registry search request was sent.

Plan of Correction: A Search of central registry will be sent for staff A.
