Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was conducted on 9/10/2020. The director was contacted by telephone to initiate the inspection. There were 67 children present and 15 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Thursday 9/17/2020. Description:	Based on record review, 1 of 4 staff records reviewed did not contain documentation of a negative tuberculosis screening or test obtained no later than 21 days after employment. Evidence: Staff #2 (Date of Hire: 12/4/19) has a tuberculosis test on file dated 2/24/2020.

Description:	Based on record review, 1 of 4 children's records reviewed did not contain all required information. Evidence: Child #3's record was missing 1 of 2 required complete emergency contacts to include a name, address, and telephone number. The complete address was missing for the second emergency contact.

Description:	Based on record review, the center did not ensure that all staff submit to a fingerprint based criminal history background check prior to employment. Evidence: Staff #2 (Date of Hire: 12/4/19) had eligible fingerprinting results dated 12/10/19. Staff #4 (Date of Hire: 9/1/20) has not yet submitted to fingerprinting.
