This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
The monitoring inspection was initiated and concluded on September 23, 2020. The Director was contacted by telephone to initiate the inspection. There were 8 children present and 3 staff. The inspector emailed the Director a list of items required to complete the inspection. The Inspector reviewed 2 children's records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violation were documented on the Violation Notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-130-A Description: Based on a review of children's records, the center failed to ensure that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
Child #1's records do contain documentation of immunizations on the first day of attendance. Immunizations were dated 08/22/2019 child's first day of attendance was 06/03/2019Plan of Correction: The center will ensure that all children will have documentation of immunizations on the first day of attendance.
Standard #: 22VAC40-185-140-A Description: Based on a review of children's records, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician within one month after attendance.
Evidence:
Child #1's records did not have documentation of a physical examination by the 30th day of attendance. Child #1's physical examination was dated 8/22/2019, child's first day of attendance was 06/03/2019Plan of Correction: The center will ensure that all children will have documentation of a physical examination by the 30th day of attendance.
