An investigation was conducted in response to an incident that occurred on 8/4/2020, and that was self-reported to the Fairfax licensing office on 8/5/2020, in regards to supervision of children during transitions. Children records and staff records were reviewed, supervision policies, Children Supervision Records, and staff interviews were conducted. A preponderance of evidence supports the report and areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-40-E Description: Based on interview, the facility did not ensure that the center's activities were maintained in compliance with the standards, the center's own policies and procedures that were required by the standards, and the terms of the current license issued by the department.
Evidence:
1.Staff #1 stated that on the day of the incident 8/4/2020, they did not follow the center's supervision of children policy which states:
- "Name-to-face attendance (visually confirming children are present by matching the names on the Child Supervision Record (CSR) to the faces in the classroom) must be taken at least every 30 minutes and during any large transitions.
- "Enrolled children must be supervised at all times. Children must not be left alone or unsupervised in classrooms, hallways, offices, lobbies, bathrooms, playgrounds, or vehicles."
- "Staff Members responsible for the children must keep the Child Supervision Record (CSR) accurate and current at all times.
2. Staff#1 did not conduct a name-to-face attendance check and updated the Child Supervision Record (CSR) before the children exited the preschool classroom and once they arrived on the center playground.
3. Staff#1 transitioned from the preschool classroom to the playground at approximately 3:15pm, during this transition one of the children remained in the preschool classroom. Child #1, age 4, was left alone and unsupervised in the preschool classroom for approximately 3 to 5 minutes.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-185-340-F Description: Based on interview and review of documentation, a child under 10 years of age was not always within actual sight and sound supervision of staff.
Evidence:
1. On the date of the incident 8/4/2020, it was determined that Child #1 was accidentally left alone and unsupervised in the preschool classroom for approximately 3 to 5 minutes.
2. Staff #1 stated that on the day of the incident they walked the preschool class from the preschool classroom to the playground at approximately 3:15pm.
3. Staff#1 stated that they did not conduct a name to face attendance check before exiting the classroom, leaving one child behind in the classroom.
4. Once Staff#1 and the rest of the class left the classroom, Child#2, age 9, attending the school age classroom, which is adjacent to the preschool classroom, observed Child#1 alone in the preschool classroom and alerted Staff#2 of the incident.
3. Staff#2 immediately walked over to the preschool classroom and escorted Child#1 to the playground to rejoin the group.
4. Based on interviews and the time when the CSR was updated the child was alone and unsupervised in the classroom for approximately 3 to 5 minutes.Plan of Correction: Not available online. Contact Inspector for more information.
