This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 9/15/2020 and concluded on 9/28/2020. The director was contacted by telephone to initiate the inspection. There were 40 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-60-A Description: Based on review of four children's records, the center failed to ensure that all required items were obtained.
Evidence:
1. Child # 2 did not two emergency contacts, did not have the phone number for the physician, and did not note any allergies.
2. Child # 4 was missing the father information, and physician's phone number.Plan of Correction: The program director will have the parents to complete all the missing information. All children's records will be reviewed and updated as needed.
Standard #: 22VAC40-191-40-C-1-A Description: Based on review of the new board officers, the center failed to ensure that all background checks were complete.
Evidence:
1. Board officer 2 and 3 took office in January 2020, a CPS check as not requested and or returned.
2. Board officer 2 and 3 did not have a completed Sworn Statement.Plan of Correction: The administration office has sent the CPS checks in and will have the board officers to complete a sworn statement ASAP.
