Standard #: 22VAC40-185-130-A Description: Based on record review and interview, the center failed obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. Child 1, enrollment date 09/08/2020, lacked documentation of required immunizations.
2. The program director confirmed that documentation of required immunizations had not been obtained for child 1.Plan of Correction: Child has physical scheduled for November. Parent will bring old shot record this week.
Standard #: 22VAC40-185-160-C Description: Based on record review and interview, the center failed to ensure that at least every two years rom the date of the first initial screening, staff members shall obtain and submit the results of a follow-up tuberculosis screening.
Evidence:
1. The most recent tuberculosis screening for staff 1 was obtained on 09/25/2017.
2. Staff 1 confirmed that an updated screening for tuberculosis had not been obtained.Plan of Correction: TB will be updated ASAP.
