This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on October 15, 2020 and concluded on October 15, 2020. The director was contacted by telephone to initiate the inspection. There were 18 children present and five staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed three children?s records and three staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-191-60-B Description: Based on review of staff records and interview, the center did not obtain an employee's completed sworn statement or affirmation prior to the person's employment.
Evidence:
The record of staff # 2, hired 03/12/2019, had documentation of a sworn statement signed and dated 04/06/2019 exceeding the first day of employment.Plan of Correction: Per the director: I feel certain that the employee completed a sworn statement at the time of hire however it can not be located. We have been creating new files lately and I believe it may be in another file. I will ensure all original documentation is kept in the file moving forward.
