This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 09/22/2020 and completed on 10/08/2020 in response to a self-reported incident involving an infant being given the wrong bottle.
Based on the information gathered which included staff interviews, review of pictures and written policies this report is found to be valid. The violation is documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-340-A Description: Based on staff interviews and electronic pictures, the center did not ensure that when staff are supervising children, they shall always ensure their care and protection.
Evidence:
1. On 09/22/2020 at approximately 12:20 pm , staff 1 fed a 2 month-old child, child 1, another child's bottle containing breast milk.
a. Administrative staff reported to the inspector, on 09/22/2020, that staff 1 had mistakenly given child 2's bottle to child 1.
2. Staff 1 confirmed that she fed child 2's bottle, containing breast milk, to child 1 and she estimated that child 1 only took in 1 or 2 sips before the mistake was noted.
a. Staff 1 additionally stated she did not check the name label on the bottle before giving to child 1.
b. The bottles for child 1 and child 2 were both dated and labeled with the full name of each child. The lid of each bottle was also labeled with the child's full name.Plan of Correction: Our policy was reviewed with the staff member immediately. An additional policy was created that now requires teachers to verbalize the child's name & the name on the bottle out loud. This policy was reviewed, trained by all staff who interact with infants.
