Comments:
A virtual monitoring inspection was completed today. Due to the COVID 19 pandemic the inspector was not on site and the inspection was conducted via mobile video application as well as email correspondence. Children were briefly observed entering the program and getting settled in after school. According to the director 20 school aged children were supervised by 5 staff. The physical plant, 3 staff records, 3 children?s records, evacuation drills, i, emergency supplies, and policies were inspected. There was a supply of materials for the children. Areas of non-compliance are identified in the Violation Notice.

Description: Based on review of child records it was determined that all required information was not maintained.
Evidence: During inspection child1 did not have parent 's work place and number.
