This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on October 27, 2020 and concluded on October 29, 2020. The provider was contacted by telephone to initiate the inspection. There were 30 children present and seven staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed three children?s records and three staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-130-B Description: Based on a review of children's records, the provider did not ensure each record contained additional immunizations once every six months for children under the age of two years.
Evidence:
The record for child #1 (date of birth 12/22/18), had one immunization record documented on 08/23/19.Plan of Correction: The immunization has been updated
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, the center did not ensure that staff records contained documentation of central registry findings within 30 days of employment.
Evidence:
1. The record of staff #3 (start date: 11/04/19) did not contain documentation of central registry findings.Plan of Correction: I emailed crs_operations@dss.virginia.gov to get another copy of the results.
