Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on October 14, 2020 and concluded on October 16, 2020. The director was contacted by telephone to initiate the inspection. There were seventy-seven children in the care of twenty-three staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed five children?s records, and five staff records, submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violation was documented on the violation notice issued to the facility. Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word `corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description:	Based on review of five staff records, facility failed to obtain a copy of the results of a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years Evidence: 1. An out of state child abuse and neglect search and a sex offender registry check for Staff #1 (DOH: 8/10/20) were requested on 8/13/20. 2. The Director confirmed that the results for both background checks had not been received. 3. The Director reported that she had not followed up within 45 days of submitting the child abuse and neglect registry search request.
Plan of Correction:	Program?s Immediate Response On 10/21/20, upon learning of the regulation to follow up within 45 days of submitting the request, the Center Director contacted the North Carolina Licensing Office to obtain the results of the one missing registry check. A return call was not received, therefore another call was placed for requested registry results on 11/18/20. An event reminder has been placed on the Center Director?s calendar to resubmit the request and follow up every two weeks. These requests will be documented for further accountability of all attempts to obtain these results. Ongoing Corrective Action Steps In response to this incident, the Center Director will place a follow-up reminder event on her calendar for 30-days after making an initial request for out of state registry results to ensure the center obtains requested results or has documentation of all attempts to receive. Conclusion The Center values our relationship with the Division of Licensing Programs and is committed to meeting all licensing requirements and operating the highest quality program. The Center took immediate and effective steps to respond to this citation, correct the deficiency and to develop a plan for the future designed to prevent further occurrences.
