This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
An unannounced non-mandated monitoring inspection was conducted on October 21, 2020 and concluded November 17, 2020, in regards to a serious injury. Investigation was conducted remotely,staff interview was conducted, video footage was reviewed and virtual observations were completed at the facility. There was no violation determined.
