This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on October 27, 2020 and concluded on November 13, 2020. A complaint was received by the department regarding allegations in the areas of supervision and care, protection and guidance. The director was contacted by telephone to initiate the investigation and a virtual remote inspection was conducted beginning at 8:30 a.m. and at 11:37 a.m. on October 27, 2020. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegations of non-compliance with standards or law and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-185-320-B Complaint related: No Description: Based on interviews, it was determined that restroom areas provided for children did not meet requirements. Evidence: Interviews revealed that children ranging in age from 13 months to 4 years of age were grouped together in the infant room. The infant room does not have a self-contained, readily available restroom and children of potty training/toileting age were taken to the adjacent classroom, through a door and across the classroom to use the restroom. Plan of Correction: Children will be placed in the appropriate classroom with access to the restroom and sink. Director will continue to pull preschool students for academic instruction/toileting until the school age room is closed.
Standard #: 22VAC40-185-340-A Complaint related: Yes Description: Based on observation and interviews, it was determined that the center did not ensure the care, protection, and guidance of children in care.
Evidence:
1. The center failed to adhere to the health and safety guidelines established due to the COVID-19 pandemic of maintaining the same groups of children day-to-day and not combining classrooms to help reduce potential exposure to COVID-19. Interviews revealed that classrooms were combined due to low enrollment. When teachers require a restroom break, all children are combined in one classroom.
2. The Licensing Inspector observed the Director carrying a 13 month old to answer the door for the arrival of an enrolled 4 year old and school ager.
3. On 10/27/2020 the Director acknowledged being aware of the guidance to not combine classrooms or staff.Plan of Correction: Manassas Park Maple Lawn plans to discontinue School age children attending the center. We will have an infant class and a preschool class. School age parents will be given a 2 week notice
Standard #: 22VAC40-185-350-F Complaint related: Yes Description: Based on interviews, it was determined that children were repeatedly reassigned to a different age group so often it created disruption in a child's schedule and attachment to his staff members and group.
Evidence:
1. Children ages 13 months through 4 years of age are grouped in the infant room. This has been occuring since August of 2020.
2. Interviews with staff revealed a four year old child reported "I don't like being with the babies".
3.When staff require a break, all children have be taken to the school age room.Plan of Correction: We will make sure the center is adequately staffed at all times to avoid grouping children together.
Standard #: 22VAC40-185-360-A Complaint related: No Description: Based on interviews, it was determined that children were not offered a variety of daily activities for all age groups that were age and stage appropriate. Evidence: Interviews revealed that children in the infant room were grouped together from age 13 months to 4 years old. Children were constantly interrupted from their daily activities to answer the door for the arrival of other children, bathroom breaks for the toileting children, lunch preparation for the center and other administrative tasks throughout the day. Plan of Correction: A lead teacher will be provided for each classroom to ensure children are engage in uninterrupted, age appropriate activities.
